Review service tiers

The NHS offers tiers of weight management. 

  • Tier 1: For everyone

    Designed to promote healthy behaviours and prevention of obesity, delivered by local and regional public health teams.

  • Tier 2: Lifestyle multicomponent weight management

    These are the first step for people wanting to lose weight. Programmes include health behaviour change strategies, can be run virtually or in-person, and in groups. Commercial providers support delivery of programmes nationally.

  • Tier 3: Specialist multi-disciplinary weight management

    These include person-led priority setting to address mental and physical health, weight loss using a range of pharmacotherapy, diet and activity approaches, and clinical oversight for medications management.

  • Tier 4: Bariatric specialist service

    This pathway can lead to surgery for weight loss, specifically to support people with severe obesity and long-term health conditions. Services offer support to people with complex physical and mental health needs.
