Re-assess status

Changing behaviours is hard.  Just like stopping smoking, it may take repeated attempts and trying different approaches to succeed at losing weight and maintaining weight lost.

Support self-management by offering repeated opportunities for referral.  There are a range of services and one size does not fit all.  How about digital or in-person, 1-2-1 or group?  Look back at the services that are available.
